Payment is made after six months, one year, and two years of employment. Summary

Assists transition patients by evaluating their self-care needs and providing the items to them after a post-acute stay. Assembles and reviews medication history and documents their findings. Hours

40 hrs/week, rotating shifts including weekdays, weekends, and holidays Responsibilities

The responsibilities of the Specialist include but are not limited to the following: Assemble medication histories by consulting patient, family members, outside pharmacies, and medical records. Document medication histories and communicate findings with pharmacy and clinical staff. Communicate patient feedback and requests to post-acute service team. Coordinate meetings, managing and delivery of the patient's self-care items. Track and monitor patient status and pending transitions to post-acute care. Engage patients to ensure that self-care needs are adequately met including medication, products, and services. Reviews medical records to evaluate the patient's self-care needs. Works with clinical team, case management, and other medical staff to achieve patient's self-care goals. Interact with patients to obtain permission to fill discharge medications, ensure patients have a method of payment, and receive counseling about their medications when needed. Deliver discharge medications in a timely manner Responsible for the procurement of all patient's information and prescriptions prior to discharge Coordinate with clinical care team, case management, and other staff to ensure medications are delivered without delaying discharge Assist pharmacist with the filling, dispensing, of patient's prescriptions Flexibility in work assignments and adaptability as patient and hospital discharge needs change Maintains a professional image and conducts behavior appropriate to the situation Comfortable working autonomously Ability to think broadly and longitudinally, constantly assessing and anticipating the needs of the patient and his/her environment Effective communication skills Participates in a culture that embraces creativity, productivity, communication, and quality improvement Ensures the Carolina Care experience is consistently delivered to every patient Ensures all operating policies and procedures are followed at the highest level Other duties as assigned Education Requirements

Completion of a nine-month Pharmacy Technician Program (or equivalent combination of education, training, and experience). Licensure/Certification Requirements

Registration with the NC Board of Pharmacy unless enrolled in a PharmD program. CPhT from PTCB or ExCPT from NHA unless enrolled in a PharmD program. Professional Experience Requirements

If completion of a Pharmacy Technician Program: No prior experience required. If a High School diploma or GED: Two (2) years of experience as a Pharmacy Technician. Prefer experience as a pharmacy technician in an ambulatory, outpatient, or retail setting. Knowledge/Skills/and Abilities Requirements
