Health Services Director - RN or LPN

Conduct and coordinate the assisted living resident assessment program in accordance with current rules, regulations, and community policies and procedures that govern resident assessment, including: Ensure an initial assisted living resident assessment is completed prior to each resident’s move-in date and a comprehensive care plan is completed upon move-in Conduct routinely scheduled re-assessments on each resident and change of condition assessments as needed to ensure residents receive the best possible care and programs Conduct interviews and observations of residents on a continual basis Alert the Executive Director, family members and others of changes in physical or emotional health and the department’s ability to meet needs Coordinate nursing services with other resident services to ensure continuity of care Collaborate with all departments to ensure residents receive services that enhance quality of life Act as a resource and care manager for residents and their families Develop and follow a monthly calendar to engage residents in meaningful activities daily Recruit, hire, train, supervise, schedule, motivate, and evaluate department staff within budget Perform administrative duties including staff evaluations, forms, reports, and resident records Coordinate prescription orders with doctors’ offices and pharmacy delivery Maintain overall safety and health of residents Aid in community marketing through positive interactions and liaison with families/outside providers Manage the department within budget, track inventory, and order supplies Ensure regulatory compliance and report issues promptly Comply with Clearwater Living standards and regulations to ensure safe and efficient operations Represent Clearwater Living principles and core values daily Perform other duties as assigned Qualifications
Minimum five to seven years of experience in senior living, healthcare, or related industry RN, LPN, or LVN certification (completed nursing coursework for certification) Minimum two to three years of supervisory experience, including training and budgeting First Aid/CPR/BLS Certification required Background clearances as required by regulations Must meet health requirements, including TB Clearwater Living is an equal opportunity employer committed to diversity and inclusion.
