Job Description Salary: From $20 per hour Overview As an Intensive In-Home Team Lead, you will deliver individualized, community-based interventions in clients homes and natural environments. You will partner with children, youth, adults, and their families to assess strengths and needs; develop, implement, and evaluate Person-Centered Plans; recommend appropriate treatment modalities; and either provide those services directly or coordinate referrals. Working under clinical supervision, youll play a key role in promoting stability and positive outcomes for clients and their support systems.

Duties Clinical & Direct Services Serve on each clients multidisciplinary treatment team and co-create Person-Centered Plans. Develop crisis and transition plans when clients enter or exit services. Deliver individual, family, and group interventionsboth in-person (70%)using modalities such as Cognitive Behavioral Therapy, Motivational Interviewing, and other evidence-based practices. Direct Services Work to implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities and other best evidence-based practice therapies. Work with clients, families, natural supports and collateral agencies in reviewing, applying and achieving goals as indicated in the Person-Centered Plan. Provide

parent/guardian/natural

support training that will aid persons interacting with the client in dealing more effectively with the client. Assist in all service delivery as formally planned with the clinical team. Facilitate child and family team meetings; train parents/natural supports in skills to reinforce therapeutic gains. Coordinate care with schools, medical providers, and community agencies; link clients to ancillary resources. 24/7 Crisis Response This position will be mostly in the community and in client's homes and may require some evening work.

Qualifications Must be licensed LCSWA or LCSW Qualified Professional- Bachelors degree in Social Work, Psychology, Human Services, or a closely related field with 2+ years of fulltime experience post degree or Masters degree in Social Work, Psychology, Human Services, or a closely related field with 1+ years of fulltime experience and Demonstrated cultural competence and sensitivity to diverse populations. Strong verbal and written communication skills; clinician-level fluency in English. Valid drivers license, reliable transportation, and personal laptop. Ability to work some evenings and spend the majority of time in clients homes or community locations. Clear background check and drug screening; free from illicit substance use.
