POSITION PURPOSE: The Licensed Therapist plays a key role in delivering comprehensive mental health services within CHI’s community health clinic. This role involves providing both individual and group counseling services to clients of all ages, utilizing a holistic approach to address substance abuse and other mental health concerns. The specialist will screen clients for depression, substance abuse, and other mental health issues, and make appropriate referrals to primary care and specialty services as needed. The position ensures a thorough and compassionate approach to addressing clients’ needs while promoting overall well-being in a supportive, federally qualified health center environment

POSITION REQUIREMENTS / QUALIFICATIONS:

Education/Experience: Graduation from an accredited university with a master’s degree in social work or Mental Health field

Licensure / Certification : Maintain current CPR certification from the American Heart Association. Licensed as a Mental Health Social Worker or Mental Health Counselor in the state of Florida. Must have a valid Florida Driver’s License.

Skills / Ability: Proficiency in verbal and written skills. Must be computer literate. Possesses knowledge of electronic health record systems. Administers PWI, FASAMS Outcome Measures, FARS/CFARS, ASAM and other behavioral health assessments measured as needed. Possesses knowledge of Confidentiality and Client Rights, HIPAA. Bilingual speakers strongly desired.

POSITION RESPONSIBILITIES (THIS IS A EXEMPT POSITION)

§ Provides care primarily for the following groups: * Infant * Pediatric * Adolescent * Adult * Geriatric § Always provides high quality of services in an outclient Behavioral Healthcare component, with emphasis on age appropriate, bio-psychosocial issues. § Provides counseling services to adults and their families using on evidence-based practice for integrated treatment of co-occurring disorders. § Participate in orientations and training related to client care with emphasis on job enhancement skills. § Provides consultation to clinicians, primary health physicians, and other staff within the BHCC. § Refers clients to Behavioral Health Services when appropriate. § Provides crisis intervention when appropriate. § Maintains a daily log of productivity and submits a weekly caseload tracking log to clinical supervisor. § Engage clients in discharge planning. § Assures that client charts are active and updated in a timely manner and they are sequentially ordered. § Facilitates daily, weekly education and counseling groups, as needed. § Conducts psychosocial assessments on clients admitted. § Monitors client’s ongoing needs, goals and progress and reflects these changes in treatment plan reviews. § Participates in daily treatment team activities as a core treatment team member. § Participates in client and family education. § Understands and applies knowledge pertaining to Community Health of South Florida, INC. Behavioral Health Services policies and procedures always. § Demonstrates knowledge of current social services practice ensuring proper judgment and decision-making guidelines. § Utilizes available time in a way is consistent and emphasizes department compliance.
