Active unrestricted license to practice medicine in any State, Territory, or Commonwealth of the United States Board Certification in Emergency Medicine (preferred) OR Board Certification/Eligibility in Internal Medicine or Family Medicine with a minimum of 2 years recent experience working in an Emergency Department setting (Experience must be within the past 5 years) BLS ACLS DEA Certificate NPI Please note: Physicians who have ever had a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
