Pride Health is currently seeking candidates for RN-Case Manager positions that will provide support for our client's healthcare systems in Fort Wayne, Indiana. Job Description:

Job Title: RN-Case Manager

Location: Fort Wayne, Indiana

Schedule: 40 hours/week

Shift: 8-hour days shifts, Monday–Friday

Contract Length: 13 Weeks

Pay Range for Traveler: $2,500.00- $2,566.00 Weekly DOE

Job Summary:

We are searching for a devoted RN case manager to coordinate and advocate for our patients' care. To this end, the RN case manager will be required to review patients' charts, and to meet with families and healthcare providers to establish the most suitable course of action. You should remain in contact with patients during these and subsequent processes.

Job Responsibilities:

• Collaborate with the care team to ensure efficient patient care and discharge planning
• Complete comprehensive patient assessments, including status evaluations to drive discharge
• Manage stay progression and discharge planning to expedite transitions to the next level of care
• Accurately document patient information in Epic
• Identify and complete tasks independently without prompting

Required Certification & Experience:

• Experience: Minimum 2 years of inpatient RN Case Management experience
• Certification: BLS
• License: Active Indiana RN License or Compact License
