Hospice RN/LVN provides nursing care to terminally ill hospice patients as needed. As an RN/LVN, you will evaluate the patient’s overall health with the goal of improving quality of life. All nursing will be performed in accordance with each patient’s individual plan of care. Responsibilities
Provide nursing services within the scope of the plan of care, in coordination with the interdisciplinary team Follow the plan of care; provide treatment; evaluate patient’s reactions to treatment Report any changes in the patient’s condition to the Hospice Clinical Director Online charting in Kinnser in a timely manner on company-provided devices Follow all company policies and procedures Perform other duties as assigned Requirements
One year of nursing experience One year of Hospice or Home Health experience (preferred but not required) Current LVN license to practice in California Current California Driver’s License
