We are a therapist-owned company serving San Diego, Riverside, and Imperial Counties, specializing in consultative and staffing services for Home Health Agencies. We seek compassionate and dedicated clinicians for flexible, per diem roles in home health therapy.
Job Summary The Per Diem Speech-Language Pathologist (SLP) provides comprehensive diagnostic and therapeutic services for patients with communication, cognitive, and/or swallowing disorders on an as-needed basis. This role offers flexible scheduling to accommodate organizational needs and the clinician\'s availability, delivering high-quality, individualized care in various settings (e.g., patient homes, clinic, facility).
Key Responsibilities
Assessment & Diagnosis: Conduct thorough evaluations using standardized and non-standardized assessments to diagnose speech, language, cognitive-communication, voice, and swallowing disorders.
Treatment Planning: Develop and implement individualized, evidence-based treatment plans based on patient needs, goals, and medical conditions.
Therapeutic Intervention: Provide direct SLP services, including therapy for articulation, fluency, voice, language, cognition, and dysphagia.
Documentation: Maintain accurate, timely, and complete patient records, including evaluations, daily treatment notes, progress reports, and discharge summaries, adhering to regulatory and organizational standards.
Patient/Family Education: Educate patients, families, and caregivers on communication strategies, swallowing precautions, and home exercise programs to facilitate progress and carryover.
Collaboration: Work with interdisciplinary team members to ensure coordinated and holistic patient care.
Compliance: Adhere to federal, state, and local regulations, professional ethics, and organizational policies.
Professional Development: Maintain licensure and pursue ongoing continuing education to stay current with best practices in speech-language pathology.
Flexibility: Be available to accept assignments as needed, including covering vacations, sick leave, or fluctuating patient volumes.
Qualifications
Experience: At least one (1) year of current experience in an acute care setting.
Skills: Critical thinking, service excellence, strong interpersonal communication, ability to read and follow instructions, organizational skills, and computer proficiency.
Disposition: Ability to learn, synthesize information, make critical judgments, work independently, prioritize patients and families, and collaborate with healthcare leaders.
Education
Master’s degree or equivalent (preferred).
Licensure/Certifications
Current California Speech-Language Pathologist License or board exam eligibility.
CCC from ASHA (preferred).
Basic Life Support (BLS) from American Heart Association (required).
AB508 Workplace Violence Prevention (4 hours) due within 30 days.
Alternative Licensure Path
Management of Assaultive Behavior (MAB 1 or MAB 2), Crisis Prevention Intervention (CPI) or other certification covering AB508 and SB1299.
Education (Preferred)
Masters or higher in Speech-Language Pathology.
Employment Type
