Overview
Health Link Home Health and Hospice Agency is seeking a skilled and experienced Hospice Community Liaison to join our team and advocate for our hospice service. As the Hospice Community Liaison, your primary responsibility will be to leverage Health Link's extensive relationships in the community to build trust in our hospice program. Hospice is about trust — to earn the trust of nurses, social workers, and doctors so they feel confident referring patients to us, you will be the front line of our hospice team and help more patients be cared for by our clinicians. You will work with the Director of Marketing and the Hospice leadership team to execute a sales and marketing plan and achieve our goals. To be successful in this role, you will need a keen understanding of the hospice industry, as well as a solid understanding of marketing and sales. Health Link has been a trusted community partner in the Bay Area for 18 years but we still operate with an entrepreneurial mindset; focus, reliability, and execution will drive success. Responsibilities
Implement a comprehensive marketing/sales plan for Health Link Hospice. Develop and maintain new and current relationships with community partners including hospital CM/SW teams, doctor offices and clinic teams, senior living communities, and skilled nursing facilities. Ride alongs with current team members, respecting existing relationships, and developing your own. Make calls to schedule meetings, visit to present our services, and perform follow-up. Collaborate with the hospice leadership team to coordinate referrals and act as the liaison between our community partners and internal staff, including messaging and coordination calls. Represent Health Link Hospice at relevant community events and meetings to build awareness and expand our network. Manage your own budget, including tracking expenses and ensuring effective use of resources. Identify, target, and develop new relationships with reliable referral partners. Perform hospice education sessions with patients and families to understand goals and preferences while evaluating fit for our services. Requirements
A passion for hospice. 1 year of experience in hospice marketing. Strong understanding of the healthcare industry, market segments, and best practices. Ability to stay organized with a CRM, calendar, and manage multiple opportunities and pipelines. Excellent communication and interpersonal skills, with the ability to build and maintain relationships. Track record of hitting targets and achieving goals. Ability to work independently and as part of a team, with strong project management skills. Valid driver’s license and clean driving record. Bachelor's degree. Benefits
