Clinical Pharmacist - Inpatient role at University of Washington (UW) Medical Center - Montlake Pharmacy Department. Job Details
40 hours per week Rotating/Variable Shift Position Highlights
Provide pharmaceutical care for our Oncology Pharmacy Satellite services, including pharmacotherapeutic recommendations to providers, nursing, and other members of the health care team to optimize patient care Responsibilities include, but are not limited to direct patient care, drug dispensing, research & education, and practice management as it pertains to the care of the assigned patient population Primary Job Responsibilities
Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies. Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed. Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations. Provide drug information and education to medical teams, nursing, and dietitian staff. Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders upon hospital admission and discharge, as well as during clinic visits. Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care between the inpatient and ambulatory settings. Monitor and modify patient medication therapy in collaboration with the medical team to achieve desired therapeutic outcomes in the ambulatory setting Provide primary preceptorship of pharmacy residents and student pharmacists. Perform the duties of a Pharmacist 2 Required Position Qualifications
Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience. Legal and Employment Notes
