Home Health LPN - PACE Program
Coverage Area: Urvada, CO Littleton, Golden, and east to I-25
Schedule: Sunday-Thursday, 7:00 AM - 3:30 PM
Caseload: 5-15 patients per day
We are seeking an experienced Licensed Practical Nurse (LPN) to join our PACE Home Health Program . This role provides direct patient care in the home setting, focusing on:

• Wound care
• Insulin administration
• Medication management and patient education

Requirements:

• Active LPN license in Colorado
• Previous home health experience strongly preferred
• Reliable transportation for coverage across the service area
• Strong clinical and communication skills
