Cedars-Sinai has a reputation for excellence and is hiring a Lead Case Manager RN to oversee the inpatient case management department within the Inpatient Specialty Program. The role focuses on coordinating care from admission to discharge in collaboration with hospitalists, CI physicians, and ambulatory teams.
Responsibilities
Assist in the daily operations of the inpatient case management department and provide oversight of daily functions of the inpatient case managers.
Conduct case management for patients in inpatient and skilled nursing facility settings.
Assess patients using CMSA model and coordinate assessment, planning, facilitation, advocacy, and discharge planning to meet health needs and achieve quality, cost-effective outcomes.
Collaborate with hospitalists, CI physicians, and health plans to determine service appropriateness and ensure quality care at a cost-effective level.
Coordinate discharge planning and alternative treatment plans with PCP/hospitalist/specialist as appropriate.
Secure outpatient follow-up appointments and arrange tests or outpatient procedures with appropriate providers.
Refer to the Ambulatory Case Manager for ongoing outpatient oversight and to avoid unnecessary readmissions.
Coordinate referrals to disease management programs (e.g., anticoagulation, asthma, diabetes, hepatitis C, smoking cessation, cardiovascular risk reduction, nutrition).
Enter and update all authorization and clinical information into Nautilus (Access Express) by discharge date.
Monitor 30-day readmissions and act as a liaison for clinical issues and workflow processes within inpatient case management.
Manage staffing schedules, respond to sick calls and absences, assist with training new hires, oversee projects, and conduct documentation audits.
Qualifications
Education:
Graduation from an accredited school of nursing; Bachelor of Science in Nursing preferred
Licensure/Certifications:
Valid CA RN license required; Certified Case Manager certification preferred
Experience:
3 years of acute nursing experience; 3 years of experience in case management, discharge planning, utilization review, and HMO in acute inpatient or outpatient settings preferred
