New Horizons is hiring a housing-focused Case Manager to engage with youth and young adults experiencing homelessness up to age 25, across the spectrum of religious beliefs, race, ethnicity, culture, sexual orientation, and gender. The Case Manager will join a direct service team and coach youth to develop their full potential, with a strong emphasis on housing solutions. This requires in-person work within group and one-on-one settings. The schedule is structured to be available to youth in shelter, engagement programs, and in the community. Each Case Manager's scheduled shifts (may occur in the morning, evening, and/or weekends) are determined through a collaborative process with the Housing Solutions Team based on need and availability. Outside of weekly scheduled shifts, Case Managers have broad flexibility to meet youth onsite, during programming, and/or in the community. The Case Manager affects change in the lives of youth and young adults (YYA) by providing a full range of case management services, including, but not limited to, engagement, assessment, intervention, short-term counseling, goal setting, and referrals for a caseload of 20-30 young people. We are looking for an organized, nonjudgemental and collaborative Case Manager to join our team. Responsibilities

Provide a full range of case management services to a caseload of 20-30 YYA, including engagement, assessment, intervention, short-term counseling, goal setting, and referrals. Work in-person with youth in shelter, engagement programs, and community settings; coordinate with the Housing Solutions Team to determine shifts based on need. Support housing-focused outcomes through individualized planning and collaboration with youth and service partners. Engage with youth across diverse backgrounds with professionalism, empathy, and cultural humility. Qualifications

Minimum 3 years’ experience in the Social Services industry. Minimum 1 year as Case Manager or equivalent in the Social Services industry. Demonstrated proficiency with PC environments including Word, Excel, and Outlook. Preferred Knowledge, Skills And Abilities

Bachelor’s or master’s degree in social work or related field. Knowledge and practice with Trauma Informed Care, Positive Youth Development, and Harm Reduction. Valid Washington State Driver's License. Fluency in Spanish or other foreign language widely spoken in King County. Proficiency with Clarity Homeless Management Information System (HMIS).
