Victorian Senior Care . Overview Mobile Crisis Management involves all support, services and treatments necessary to provide integrated crisis response, crisis stabilization interventions and crisis prevention activities. Services are available 24 hours a day, 365 days a year, providing timely evaluation, triage and access to acute mental health, developmental disability and/or substance abuse services. The program also provides treatment to support symptom and harm reduction and safely guides people in acute crisis to appropriate crisis stabilization and detoxification support/services. These services include immediate telephone response to assess the crisis and determine risk, mental status, medical stability and appropriate response. Responsibilities
Assist and supervise Mobile Crisis Management employees in providing 24/7/365 crisis response.
Provide immediate telephonic assessment of crisis, determine risk and mental/medical status; provide face-to-face response as needed.
Develop a Crisis Plan for recipients new to the public service system and update existing crisis plan components in Person Centered Plans, as appropriate.
Monitor and manage presenting psychiatric and/or addiction symptoms; maintain proper documentation and authorizations.
Coordinate movement across levels of care, discharge planning and community re-entry following hospitalization or residential services to ensure continuity of care.
Maintain training requirements and forward required documentation to designated staff in a timely manner; ensure proper authorizations for all Mobile Crisis Management services.
Support the dignity and safety of recipients; work cooperatively with staff and relate to recipients and families.
Attend mandatory staff meetings and consumer conferences or provide advance notice for inability to attend.
Receive supervision from the Mobile Crisis Management Director or Designee; report changes in consumer condition, family situation or needs, and any accidents or incidents to leadership.
Report any suspected abuse, neglect or exploitation to the Mobile Crisis Management Director or Designee.
Be familiar with TA, Inc. policies and procedures including emergency services and medication administration; comply with HIPAA regulations and protect consumer rights and confidentiality.
Operate office equipment as necessary and maintain current training as required by the agency.
Understand that failure to renew required trainings by due date may affect scheduling.
Perform other duties as required by circumstances.
Qualifications
Graduate of a college or university with a bachelor’s degree in any field other than human services and four years of full-time, post-bachelor’s degree MH/DD/SA experience with the served population, or a substance abuse professional with four years of full-time post-bachelor’s degree supervised experience in alcoholism and drug abuse counseling.
Must have a valid NC Driver's License and at least eight years of driving experience (per company liability insurance).
Minimum of one year’s experience providing crisis management services in settings such as assertive outreach, assertive community treatment, emergency department or other 24/7 crisis response settings.
QP status and prior supervisory experience in the mental health field.
Ability to Lift/Push/Pull a minimum of 25 pounds.
