Overview San Diego’s St. Paul’s PACE program (Program of All-Inclusive Care for the Elderly). This innovative program serves individuals 55 years or older who are living with chronic illness or disabilities and need coordinated medical care to remain independent at home and in the community. The healthcare teams at St. Paul’s PACE provide hands-on coordination of medical, social, and home care services so individuals no longer need to manage their medical care alone.

Job Summary Under the supervision of the Social Work Manager, the MSW lead will provide supervision, training, and support to staff in the social work department to ensure PACE Policies and Procedures, as directed by regulatory agencies, are accurately and efficiently carried out by staff on a daily basis.

Qualifications and Experience

Education:

Master’s Degree from an accredited school of Social Work required

Experience:

A minimum of two years’ experience working with a frail or elderly population required. Experience working on a multidisciplinary team in a hospital, nursing home, or community-based setting preferable. Mental health and substance abuse experience preferred. Shall have either training or related experience in the job as signed.

Duties and Responsibilities

Manage a caseload of participants ensuring high-quality service delivery and effective case management.

Provide support to the SW Manager.

Review and approve case plans, assessments, and documentation to ensure accuracy and compliance for BSW and SWSS staff.

Collaborate with the SW Manager to provide support and advocacy to ACS partners.

Assist in the development and implementation of policies and procedures related to social work services.

Assist with onboarding and training of new staff for the social work team.

Oversee required meetings such as Participant Advisory Committee, Residential Advisory Committee, and Pat Ford Fund.

Facilitate an Interdisciplinary Team and Plan of Care meeting daily.

Work with the Enrollments Department to assess new referrals and make recommendations for enrollments.

Coordinate with the interdisciplinary team to develop a comprehensive care plan for each participant.

Conduct in-person reassessments of enrolled participants every six months and as needed.

Act as liaison with participants, caregivers, and community agencies regarding orientation to and ongoing relations with the interdisciplinary team, day center, and other St. Paul’s PACE staff.

Provide ongoing support, counsel, and education to participants and family/significant others regarding aging, dementia, grief and loss, end of life, disease processes, difficult family dynamics, PACE model and services, mental health education and treatment, and substance abuse education and treatment.

Work to access and maintain participant housing through intervention with participants and caregivers, partnering to maintain appropriate housing and promote independence where possible.

Perform home visits as needed to assess living environment and support system.
