Sr Family Advoc Patient Family Centered Care NES
The Sr. Family Advocate serves as a member of the Patient and Family Centered Care team, promoting Patient & Family Centered Care across the continuum of care. Provides support and advocacy for patients and their families throughout the entire treatment cycle in all inpatient and outpatient areas. RESPONSIBILITIES: Provides direct services such as providing support, resources, and education to patients and families. Provides initial consultation with and orientation for specific pediatric populations and their families. Provides support in assisting families in coping with the stress of treatment and care necessary for patients. Educates parents regarding their role on the health care team. Functions as a Sr. Family Advocate within role as a member of pediatric team. Assesses and identifies family-centered needs on individual and systems levels. MINIMUM QUALIFICATIONS: Bachelor’s degree preferred or the equivalent of life and work experience. Life experience as a parent of a child with a life-threatening illness or developmental disability or inpatient hospitalization required. High level of interpersonal skills to function as a member of an interdisciplinary team. Demonstrated knowledge and skills necessary to provide care to patients 0-21 years of age.
