Overview

The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include, but are not limited to: drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department of Pharmaceutical Services is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.D. curriculum, clinical research, and public and clinical service. Pharmacy - Parnassus Meds Job Summary The Oncology and Infusion Staff II Pharmacist provides pharmaceutical care to patients in all areas where pharmaceutical services for Infusion/Adult Hematology/Adult Oncology are provided within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Pharmacist performs a wide range of pharmaceutical care functions to ensure drugs prescribed for and used by patients are safe and effective. Responsibilities include obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drug choices and therapeutic plans, ensuring access to medications, educating patients on proper use of medications, and ensuring patient understanding of cautions, adverse reactions, and interactions related to therapy. The Pharmacist will be part of an interprofessional team with physicians, advanced practice providers, nurses, and other professionals. The Pharmacist will be based in the ambulatory or inpatient setting, managing injectable, oral, and/or infusion medications for patients with hematologic and oncologic malignancies. Depending on the assigned shift and site, the Pharmacist may oversee pharmacy technicians for sterile compounding or facilitate insurance authorizations for medications. The Pharmacist may also oversee clinical activities, delivery of services, and the Patient Management Program as applicable. Staff are expected to participate in department initiatives to improve quality and safety. This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs. Compensation ranges online vary; offers typically fall in 80% - 120% of the established mid-point, based on experience and current salary in similar roles. Required Qualifications Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy Preferred Qualifications Completion of an ASHP-accredited PGY1 Pharmacy Residency and/or Fellowship, or at least 3 years of Oncology/Infusion experience Completion of an ASHP-accredited PGY2 Oncology Pharmacy Residency and/or Fellowship, or at least 3 years of Oncology/Infusion experience Advanced board certification (e.g., BCOP, BCPPS, or BCPS) Operational experience in Oncology and/or Infusion Pharmacy Experience in sterile compounding of hazardous and non-hazardous drugs Licensure/Certification Must possess a current license in good standing to practice pharmacy in the State of California Current certification for Basic Life Support (BLS) from the American Heart Association About UCSF At UCSF Health, our mission of innovative patient care, advanced technology and pioneering research is redefining what’s possible for the patients we serve.
