Nurse Practitioner - Primary Care physician job in Illinois: Nurse Practitioner - Cook County suburb- 10 minutes from downtown Chicago- Outpatient Monday to Friday - 8a-5p- No nights, No weekends! Visits: 20 min. regular visit/ 40 min. full diagnostic and new patient Seeing 13-16 patients a day Physicians care team: Scribe, MA, Social Worker Equipment at clinic: Labs on site with a phlebotomist/ access to RubiconMD (access to specialist) Will be supported by the Medical Director in a value based care model for preventative health, concentrating on adults- with a large patient base which is bilingual Spanish Salary Range for both is 105,000-135,000 (Experienced provider with bonus can make up to $155k) Annual clinical performance bonus (DOE between 15k & 25k bonus) Health & Dental insurance Paid time off Relocation assistance Vision insurance Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate Bilingual Spanish strongly preferred Ability to obtain and maintain Illinois license 1 year experience preferred plus national certification in at 1 of following specialties: Family Nurse Practitioner Adult-Gerontology Primary Care Nurse Practitioner Adult Nurse Practitioner Gerontological Nurse Practitioner Active, non-probationary state Nurse Practitioner license Active DEA license
