We are currently seeking a friendly, patient-focused, and detail-oriented Associate Care Team Coordinator (Care Team Support) to join our dynamic team. As an Associate Care Team Coordinator, you will play a vital role in completing essential processes and protocols. You will collaborate closely with all members of the care team to uphold the organization's vision and mission, ensuring excellent customer service and adherence to Lean processes. Your support will be instrumental in achieving our financial, clinical, and service goals.

5 Neponset St. Worcester, MA ( Primary Care Department - Third Floor ) Schedule (38.75 Hours):
Monday-Friday, 8am-4:30pm and 8am-5pm EST. The schedule may vary based on business needs. Main Responsibilities: Enter orders and referrals, and sort faxes as requested by the primary care provider or care team. Process prior authorizations. Collect or verify demographic information, including key demographic fields. Update and load insurance information as necessary. Verify eligibility and flag appointments appropriately. Schedule appointments following established guidelines. Utilize call processing and EHR functionalities to take messages and communicate effectively with clinicians. Monitor daily reminder call logs; reach out to patients needing appointments; follow up with patients who "no show" for follow-up appointments focused on chronic conditions or preventive care. Manage administrative tasks in the EHR in-basket and respond in a timely manner. Scan and index documents into the EHR; ensure all necessary documents and lab results are accessible before patient visits. Assist coworkers as needed to maintain smooth office operations and deliver excellent service as a team. Exhibit respect for patients and staff by maintaining a friendly and courteous demeanor at all times and ensuring patient confidentiality. Perform other duties as assigned. Required Qualifications: High School Diploma/GED (or higher). 2+ years of customer service experience. 1+ years of receptionist experience. Preferred Qualifications: 1+ years of experience as a receptionist in a healthcare setting or 5+ years of customer service experience. Experience with electronic health records (EPIC).
