ASSIGNMENT SUMMARY
The Patient Care & Outreach Coordinator plays a vital role in enhancing patient engagement, access to care, and overall satisfaction. This position supports clinic operations by coordinating outreach initiatives, ensuring continuity of care, and promoting health education and preventative services. The coordinator works closely with providers, medical assistants, community partners, and administrative teams to serve diverse populations with compassion and professionalism.
MINIMUM QUALIFICATIONS High school diploma required, associate or bachelor's degree in healthcare, public health, or related field preferred Preferred experience in health education and outreach Familiarity with EHR systems and Microsoft Office Flexible work schedule; may be required to work some nights and weekends Computer proficient ESSENTIAL FUNCTIONS/RESPONSIBILITIES
Ensure patients receive timely follow-up care, referrals, and support services, including behavioral health and social services Provide State/Federal enrollment assistance for underinsured or uninsured community at large and grow unique Kheir patient population. Monitor clinic appointment adherence and conduct outreach to patients with missed visits, using culturally competent and trauma-informed approaches Assist with patient care navigation, including coordination of transportation, interpreter services, and support for patients with limited health literacy Identify high-risk patients and collaborate with care teams to ensure access to continuity of care Maintain accurate documentation and update electronic health records (EHR) accordingly, including progress notes and outreach logs. Develop and implement outreach strategies to increase patient access, retention and utilization with emphasis on underserved and vulnerable populations Represent the health center at community events, health fairs, and partner meetings as a liaison for clinical and support services Build relationships with local organizations, schools, shelters, and advocacy groups to identify unmet needs and facilitate service referrals Lead or support coordination of targeted quality measure outreach campaigns (e.g., screenings for patient) Maintain inventory of outreach materials and support translation/adaptation for multilingual audiences Facilitate patient education on topics such as chronic disease management, preventative screenings, and social determinants of health Support vaccine campaigns, health screenings, and wellness initiatives in coordination with clinical teams Track patient responses and adapt educational materials based on feedback and population needs Track outreach activities, outcomes, and patient engagement metrics for reporting and quality improvement Maintain timely and accurate logs to support grant deliverables, compliance tracking, and operational planning Assist in preparing materials for community outreach and health education, including flyers, slide decks, and social media content Collaborate with data teams to review reports and identify gaps in outreach or care coordination Participate in interdisciplinary meetings to evaluate performance and propose process improvements Maintain confidentiality in handling patient information and adhere to HIPAA regulations and internal data privacy protocols Participate in training on compliance, cultural humility, and safety procedures relevant to outreach settings Support onboarding and training of new staff engaged in outreach or patient navigation Other duties as assigned.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
This is primarily an office position that requires only occasional bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. The position requires daily use of a company provided computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper. The position requires occasional travel to attend on- and off-site meetings.
