QIDP-Residential Coordinator (Healthcare)

FUNCTION: The Qualified Intellectual Disability Professional reports to the Program Director, DC-IDD and is responsible for the day-to-day operation and customer service of the residential facilities. This includes supervising and training the Direct Support Professionals, available by phone 24 hours a day, seven days a week, to ensure around-the-clock safety and supervision. Provides day-to-day supervision of DSPs to ensure their duties and responsibilities are completed. Manages appointments and day program drops, as needed, maintains house/program budget, schedules staff to limit overtime, works shifts as needed to meet staffing ratio needs, and maintains a safe and orderly program in accordance with the Scope of Work and Volunteers of America Chesapeake policies and procedures.

RESPONSIBILITIES Physical Plant and Safety

Ensure the interior and exterior of each program site are maintained in a safe, clean, orderly, attractive, and sanitary manner and free of dirt, rubbish, and objectionable odors.

Identify environmental risk issues and work cooperatively with Direct Support Professionals.

Work with Direct Support Professionals to ensure residential program environments are clean, safe and free of hazards; participate in the resolution of maintenance concerns.

Complete monthly environmental inspections/reviews as outlined in VOACC Quality Assurance Plan.

Develop Personal Emergency Preparedness Plan (PEPP) for each person served and ensure it is reviewed quarterly; train staff as needed on the plan.

Assist in recruiting and hiring staff for homes in accordance with budgeting and licensing guidelines.

Schedule initial and ongoing training; monitor expiration dates of key training.

Maintain an orderly documentation system of all training and in-services, supplemented by an electronic database and/or spreadsheet.

Ensure Direct Support Professionals are trained on Individual Service Plans and Individual Financial Plans.

Address performance issues in accordance with organizational policy; implement employee recognition efforts; mentor staff and persons receiving services.

Training of Direct Support Professionals

Ensure DSPs are trained on Persons Support plans and goals by assisting with the development of competency-based trainings and examinations.

Participate in monthly house meetings and conduct training as needed.

Mentor staff and persons receiving services; promote a stable work environment to encourage productivity.

Conduct and participate in trainings that teach best practice models and evidence-based outcomes.

Work with DSPs to ensure documentation is appropriate in progress notes and other forms.

Ensure staff working with persons supported receive person-specific training on all current documents and required support relevant to the service provided.

Provide needed training to the day service provider; train staff on monitoring and reporting adaptive equipment issues.

Assist Program Director in planning and conducting new hire orientation sessions.

Design and Development of Person Centered Thinking/Practices

Collect and review information, assessments and reports to support development of individualized person-centered plans; ensure clinical documentation is in place within designated time frames.

Coordinate the clinical component of services, including monthly/quarterly reports and updates to personal information.

Ensure Person Centered Plans meet the hopes, dreams, aspirations, and capabilities of the supported persons and focus on outcomes.

Participate in the Person-Centered Plan Team; ensure progress is documented on ISP goals/objectives and DSP-led goals.

Ensure quarterly reports meet required standards and are uploaded as outlined in waiver rules; ensure services align with waiver prior authorization and ISP.

Ensure all clinical therapies (Nutrition, Speech, PT, OT, etc.) assessments and quarterlies are completed timely and recommendations implemented as written.

Coordinate monitoring of adaptive equipment and link persons to services and resources; coordinate with external entities to maintain program excellence.

Medical and Health Services

Audit MAR weekly for quality and appropriate medication dispensing; complete medication error reports as necessary.

Ensure specialized support elements (mobility devices, assistive tech, vision/hearing, dental devices) are available and in good working order.

Ensure all medical/health assessments are current; timely review medical consultation reports and recommendations/appointments.

Inform and maintain communication with service coordinators and families on medical concerns, illnesses, injuries and incidents.

Nutrition, Healthy Living, and Positive Self Image

Work with RC to ensure persons supported experience good nutrition, healthy living, and a positive self-image.

Ensure choices regarding nutrition and health activities; assist DSPs with groceries aligned to nutrition plans and mealtime protocol.

Community Connections and Celebrations

Design and pursue community integration opportunities aligned with person-centered goals.

Connect people with community resources; recognize achievements and celebrate birthdays/holidays.

Communication

Maintain a positive image inside and outside the organization; establish effective communication with persons, families, and staff.

Ensure positive language and respectful interactions; review logs and communicate progress with day programs and contractual services.

Practice active listening; use multiple communication forms; ensure benefit/entitlement programs are maximized (Medicaid, Medicare, etc.).

Manage financial accounts with Administrative Assistant; ensure billing census sheets are accurate and timely; ensure expenses are reflected in IFP; perform monthly financial reviews; collect receipts within 72 hours; monitor spenddown as applicable; review IFP ongoing; assist persons in budgeting.

Records Safety, Security, and Confidentiality (Qualitative Approach)

Conduct monthly Quality Assurance audits as outlined in VOAC Quality Assurance Plan to evaluate service quality and initiate change when warranted.

Ensure records are accurate and up to date; maintain confidentiality and HIPAA compliance; ensure clinical records meet compliance standards.

Behavioral Programming Development Assistance/Data Interpretation

Support development of individualized behavioral support services; ensure data is reviewed and interpreted.

Complete all action steps of Behavior Support Plan in MCIS for DDS RCRC approval; recognize behavior as communication and improve communication efforts.

Train staff on behavioral interventions and documentation; follow up with BSP clinician for training as needed.

Rights, Respect, Persons Advocacy

Demonstrate respectful interactions; assist in Human Rights Committee documentation as needed; protect rights and minimize restrictions.

Facilitate advocacy for people supported; ensure accurate incident reporting and cooperation with investigations.

Support staff as mentors and teachers; ensure mandated reporting responsibilities; monitor environments for abuse/neglect indicators.

Complete annual Abuse, Neglect risk assessments and develop prevention plans; train staff on Prevention Plan actions.

Licensing/Monitoring/Funding/Accreditation Compliance

Ensure regulations follow organizational and state standards; staff are familiar with standards.

Work with Community Services Boards and other agencies; assist Director in responding to plans of correction and obtaining required documentation.

OTHER:

Travel may be required; valid driver’s license; vehicle liability coverage if driving personal vehicle.

Background checks (Education/Work History, OIG Exclusion), negative drug screening; CPR/First Aid certifications required and maintained.

Operate a motor vehicle safely to transport self, consumers, and program supplies as necessary.

Travel to assigned locations; stand, stoop, bend, reach, pull, push, lift, talk, see, hear; perform basic and light home maintenance and operate office equipment.

Move and lift light objects up to 30 pounds; continuous or repetitive hand/arm movements for office equipment.

Remain in a sitting position for extended periods.

EFFECT ON END RESULTS:

Ensure client needs identified by the IDT are met and client sanitation is achieved.

Operate the program in compliance with contract scope of work, VOA Chesapeake/Carolinas policies and licensing/Medicaid Waiver regulations.

Support persons as participatory community members; design schedules flexibly to meet clinical demands with Program Director approval.

REQUIREMENTS

Bachelor's degree in human services field.

Minimum 1 year of experience working with people with intellectual or developmental disabilities.

Ability to participate in weekend on-call rotation.

Current CPR/First Aid Certification and maintained once employed.

Must be a certified TME or obtain TME certification within 6 months of signing.

Attend training activities, meetings and routine supervision with the Director of Operations.

Excellent computer skills (Microsoft Office); strong written and verbal communication skills.

Proven ability to resolve conflicts; bilingual in any language is a plus.
