The Branch Clinical Director is responsible for ensuring that patient care and services is coordinated, delivered, and managed appropriately. Essential Job Functions/Responsibilities Reviews case referrals. Reviews available patient information related to case, including disciplines required to assist in determining home care needs. Assists in assignment of appropriate clinicians to case. Reviews and evaluates each case by reviewing the services provided by clinicians, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. Reviews patient’s clinical diagnosis, medications, procedures and clinical course. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. Leads case conference meetings with agency personnel to facilitate coordination of care. Conducts record reviews and communicates findings and recommendations to agency personnel. May assist in the screening and interviewing process of new agency personnel and make recommendations for the employment of individuals. Assists in the orientation of new agency personnel. Assists in the planning, implementation and evaluation of in-service and continuing education programs. Works hand-in-hand with the Director of Nursing on agency QAPI. Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives. Complies with accepted professional standards and principles. Promotes customer service orientation to all agency personnel. Performs other duties and activities as assigned. Position Qualifications Registered Nurse with current licensure to practice professional nursing in the State. Prefer two years management or supervisory experience in health care setting, preferably home care. Complies with accepted professional standards and practice. Must be a licensed driver with an automobile that is insured in accordance with state and Organization requirements and is in good working order. Possesses and maintains current CPR certification. Demonstrates excellent observation, verbal and written communication skills.
