Emergency Department Clinical Pharmacy Specialist

Banner University Medical Center Phoenix , you will be responsible for developing and maintaining a high-level clinical pharmacy practice in Emergency Medicine. Patient care responsibilities include ensuring appropriate therapy for ED and Observation patients and completing and managing pharmacokinetic, anticoagulation, and pain consults. Expected to participate in educational initiatives and staff development; develop and provide oversight for the quality assurance of clinical pharmacy programs that improve patient care and experience; and participate on system and hospital committees and pharmacy and therapeutics committee as assigned.
PGY2 in Emergency Medicine or Critical Care or equivalent years of clinical practice in the Emergency Department is preferred. This is a full-time position working 10-hour evening shifts from 11:30am-10:00pm, with an 8 day on 6 day off rotation, Wednesday-Wednesday. Schedule subject to change based on business needs and on rare occasions overnight coverage may be required. Position Summary Actively participates on the healthcare team as a pharmacotherapy expert to provide quality patient care in area of specialty. Participates in the optimal selection of drug therapy and designs a regimen to maximize therapeutic benefit and minimize toxicity. Critically evaluates the management of disease states and adjusts or recommends adjustments to therapy to improve patient medication regimens. Drug information expert in area of specialty and may act as a system resource. Reviews and evaluates medication orders for appropriateness and completeness with respect to patient data. Applies the principles of continuous quality improvement consistent with job expectations. Participates in local and system-based committees and effectively communicates to stakeholders. Meets ASHP preceptor qualifications. Minimum Qualifications A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program. Board certification through the Board of Pharmaceutical Specialties (e.g. BCPS, BCOP). Requires pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. Preferred Qualifications Completion of a PGY2 residency or PGY1 residency plus a minimum of 3 years of health system experience. Board Certification in appropriate area of specialty.
