206 ($67,000) OUTCOMES Responsible for the coordination and integration of all Social Services ministries among Salvation Army units and resources within an assigned region of the City of Chicago. Responsible for the full implementation of the Pathway of Hope model within the assigned region.
Essential Duties And Responsibilities
Administration and Planning
Team Development: Provide professional direction to coordinate all program ministries within the service region.
Program services are delivered and coordinated in a fully integrated manner within the context of overall corps ministries.
Ongoing review and evaluation of program outcomes to improve service delivery.
Pathway of Hope and Case Management Support
Manage the implementation of Pathway of Hope throughout the region, including staff training and review of outcomes.
Provide ongoing supervision and administrative oversight for the region in the implementation of Pathway of Hope.
Ensure quality of local data and reporting to support national and territorial POH outcomes.
Communication
Maintain excellent communications within the Region and Division.
Weekly meetings with officers and program staff; regular meetings with Divisional POH Project Manager and Regional Team to review City Mission strategy.
Collaboration
Establish and maintain collaborative relationships across regions to meet mission objectives.
Support team building at Divisional POH Meetings, Regional Meetings, and Corps Officer Meetings.
City Mission and Divisional POH program support
Participate in City Mission Management Team; attend monthly meetings; share updates with Program Managers and POH Coordinators.
Provide ongoing support to City Mission social service programs and POH leadership; participate in QA reviews and provide guidance during critical times.
Resource Development
Strengthen program ministries by seeking additional resources.
Provide community resources at Regional and Divisional meetings; ensure POH planning quality at sites.
Staff Development
Provide professional support to staff with training and resources; conduct workshops as requested.
Clinical Intervention/Case Management Support
Consultation for Social Services Coordinator and Corps caseworkers on crisis intervention and case management; guidance in mandated reporting and mental health issues.
Plan, coordinate and facilitate monthly Corps caseworker meetings; meet with staff and Officers monthly to discuss community linkages and data needs.
Intra-Agency Responsibilities
Ensure relevance and consistency of social services by collaborating with other Salvation Army units; adhere to policies and objectives with outcome-based measures.
Assist POH scaling and learning agenda with key project managers and regional coordinators.
Community Relations
Attend inter-agency meetings and maintain relationships with community agencies to provide comprehensive services and stay informed on trends and resources.
Continuing Education
Participate in professional seminars and conferences; maintain active involvement in the social work field and incorporate new developments.
Prepare materials for presentation or publication as approved; stay current on legislation affecting social work practice.
Plan and participate in POH workshops; maintain POH personnel database; supervise social work interns as requested.
Reporting Relationships This position reports to the Director of City Mission and collaborates with Divisional POH Project Manager, program staff, corps officers, and social services to ensure effective service delivery.
Performance Measurements Evaluated on achievement of outcomes, timeliness and accuracy within budget, and 30/60/90-day reviews.
Education/Experience
Master’s degree from an accredited school of Social Work.
Licensed Clinical Social Worker (LCSW) preferred.
3-5 years in social work with experience in service delivery and staff supervision.
Bi-Lingual, Spanish preferred.
Competencies
Commitment to The Salvation Army mission; strong knowledge of social work principles, counseling, and case management.
Ability to integrate social work with organizational goals; strong communication and collaboration skills.
Respect for diverse client populations and ability to handle confidential information.
Position Limitations

Physical Demands/Work Environment
Moderate physical work; ability to lift 10 pounds; use of computer and office equipment; office with low noise; travel to sites in the Chicago area.
