Overview We are hiring a full-time Substance Use Disorder Counselor to join our dynamic care team at Southeast Rehabilitation & Skilled Care Center in North Easton.

Experience & Education

Bachelor’s or master’s degree in psychology, counseling, social work or related field.

Preferably licensed to practice in the state (LMHC/LICSW/LSW/ LMFT/ LADC-I).

Experience in the treatment of clients with a substance use disorder.

Duties & Responsibilities

Provide direct services to residents as represented in the caseload through evaluations and screenings.

Conduct crisis/emergency assessment and intervention.

Develop and implement treatment planning and assessment; provide individual and group therapy.

Evaluate residents’ biopsychosocial needs and identify barriers to medical rehabilitation.

Facilitate individual and group counseling; develop appropriate, measurable treatment plans for SUD.

Participate in interdisciplinary care plan meetings and collaborate with doctors, staff, social services, and external partners.

Maintain client and administrative records according to Athena’s policy.

Attend administrative meetings and provide ongoing inservice education on SUD care and related topics.

Perform risk assessments using current tools to determine appropriate level of care.

Engage clients with clinical expertise specific to the target population (substance abusers in SNF) and coordinate discharges (e.g., detox, residential treatment, IOP/SOAP, MAT).

Handle related duties as assigned.
