About PACE Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.

One Team One Mission Position Summary The Center Manager is responsible for the oversight of the Gary and Mary West PACE nutrition, home care, and personal care services. In addition, the Center Supervisor will assist in maintaining a high-functioning Interdisciplinary Team (IDT) and coordinating the care planning process for the participants. The Center Manager will assist the Center Director in facilitating the Participant Advisory Committee (PAC) meetings. The Center Manager facilitates collaborative relationships across Gary and Mary West PACE teams and stakeholders to promote the Gary and Mary West PACE values, team culture and mission.

Essential Functions

Participate as a member of the interdisciplinary team (IDT); participate in care planning: initial, unscheduled, and annual assessments of participants as needed.

Assist in the supervision of day center, homecare, and dietary staff.

Assist in the hiring, training, supervising, and mentoring of assigned staff.

Coordinate, schedule, and facilitating PAC Committee meetings.

Work in collaboration with the Recreation Supervisor to facilitate New Participant Orientation.

Collaborate on the GMWP Newsletter with the Recreation Supervisor, providing edits and guidance before finalization.

Develop and oversee a PACE Ambassador program for new incoming participants

Assist with the development, oversight, implementation, and adherence to policies, procedures, and standards for all services in consultation with other management staff.

Provide support to Dietary team, Home Care Coordinators and Personal Care Attendants on the day center floor.

Complete all required documentation in a timely and accurate manner.

Protect privacy and maintain confidentiality of all company procedures and information about employees, participants, and families.

Follow Gary and Mary West PACE policies and procedures and participate in any required Quality Improvement activities, committees, projects, staff training and meetings as assigned.

Be flexible in the schedule of hours worked.

Other duties as assigned.

Adhere to and support the center's policies, practices, and procedures.

Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.

Skills and Experience

High Minimum of one (1) year of documented experience working with the frail elderly population.

Minimum of five (5) years of supervisory experience in a medical setting.

Skilled in medical terminology, including knowledge of the appropriate documentation for completing assessments.

Ability to establish and maintain effective communication with colleagues, participants, and other necessary parties.

Education and Certification

Bachelor's degree in health care administration, nursing, public health, or related human services field.

Medical Clearance Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
