As a
Per-Diem Occupational Therapist , you’ll provide
personalized therapy in a home setting , helping patients regain independence in
daily living activities
through customized
treatment plans and adaptive strategies .
Key Responsibilities: Conduct
comprehensive patient assessments
and establish
realistic therapy goals .
Develop
personalized treatment plans
focused on
ADLs, motor skills, cognitive function, and environmental modifications . Provide
hands-on occupational therapy services , helping patients
increase mobility and functionality . Educate patients and caregivers on
adaptive techniques, energy conservation, and safety strategies . Collaborate with
nurses, physical therapists, and other healthcare professionals
to provide
comprehensive care . Maintain
accurate and timely documentation
following
regulatory standards . Stay current on
home health regulations and evidence-based therapy techniques . Qualifications:
Valid State License
as an Occupational Therapist Minimum 1 Year of Experience
in home health or acute care Strong
clinical skills
with a patient-first mindset CPR Certification
(Required) Excellent
communication & interpersonal
abilities Reliable
transportation
for home visits Understanding of
