Job Summary:

As the CIN Care Navigation RN, you will coordinate and optimize care transitions for ACO and patients transitioning from acute care to post-acute care and ultimately, home.

You will track patients transitioning into post-acute care facilities to ensure continued quality care. You will work with post-acute providers to monitor and improve patient outcomes.

REQUIREMENTS

Education:Associate or bachelor's degree in nursing.

Skills:Must have knowledge of case management and community resources. Must be able to function independently and effectively in a fast-paced environment. Written and verbal skills are essential. Must be able to establish priorities and communicate effectively.

Years of Experience:At least 2 years of previous case management, care navigation, or post-acute care experience.

License:Must have active and unencumbered RN in Ohio and Michigan - or be willing to obtain upon hire.

PREFERRED REQUIREMENTS

Education:Master's degree in nursing or other relevant healthcare field.

Skills:Microsoft Office competency
