The Patient Engagement Specialist

has a background in medical assisting

and draws upon their clinical knowledge to work directly with patients in developing, implementing, and monitoring virtual programs on the GetWell Network to support their care. This role requires the use of analytical and communication skills to educate patients, advocating for the use of the GetWell Network to increase the effectiveness of care delivered by our facility. This individual must possess an understanding of the differing socio-cultural and health perspectives of each patient to effectively enroll patients into the network and to monitor how patients can best use the network to improve their self-care. RESPONSIBILITIES: The Patient Engagement Specialist will be able to draw upon

prior medical assisting or back office clinical experience

to assist patients in their pre- and post-care to: Assess patient interest in technology and their ability to use devices to access their patient portal Assist patient in portal enrollment, educating the patient on portal features and how to navigate them Assist patient with installing apps (patient portal, remote patient monitoring, etc.) on their device(s) Assist patient in using prescribed remote monitoring devices and related software Troubleshoot patient ability to utilize patient portal, remote monitoring devices, and related software Utilize technology/software to conduct satisfaction surveys and enter data General Duties: Draw upon prior experience in medical assisting

to communicate patient care instructions in a verbal or written manner. Observe and comply with all EHR Program and IHMP policies and procedures including patient privacy and confidentiality, computer security and access controls, and appropriate conduct befitting a healthcare employee when engaging with patients, families, staff, and vendors Identify opportunities and risks and provide associated recommendations to EHR IT Director in a timely manner Create and maintain timely documentation of work activities and workflows; and prepare written summaries of the status of patient engagement on a monthly basis Demonstrate professional standards in documentation and communication with patients and coworkers; apply effective strategies to avoid conflict with workplace staff and patients and employ constructive methods to resolve conflicts should they arise Clearly articulate problems, issues, and potential solutions to IHMP administration and EHR Team members in a timely manner Demonstrate ability to balance multiple work streams and associated complexity while achieving program goals Seek assistance as needed Other duties as assigned Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. QUALIFICATIONS REQUIREMENTS: Prior experience in medical assisting or equivalent position required Knowledge of basic computer/technology skills—download and troubleshoot apps, create emails, cell phone and/or computer setup/functioning, Internet searching, etc. Excellent oral and written communication skills Demonstrated aptitude for patient advocacy, empathetic program design, cross-cultural communication and understanding of the role of social determinants in self-management Working knowledge of health-related business processes Demonstrated ability to lead and to work independently Demonstrated ability to produce professional communication for a relevant range of audiences EDUCATIONAL REQUIREMENTS: High School Diploma or equivalent, required Medical Assistant Diploma, highly preferred A concentration in health science education or allied health field, preferred TYPICAL PHYSICAL DEMANDS Position requires prolonged sitting, some bending, stooping, and stretching. Good eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment is also required. Employee must have normal range of hearing and eyesight to record, prepare, and communicate appropriate reports and may need to lift up to 30 pounds.
