Occupational Therapist (OT) - Home Health, Marin County role at Focus Home Health - San Jose. Salary: $55.00 - $65.00 per hour.
Overview As a Home Health Occupational Therapist, you will evaluate and treat patients with injuries, illnesses, or disabilities in their homes. You will help patients recover, improve, and maintain skills needed for daily living.
Responsibilities
Evaluate patient functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.).
Evaluate home environment for hazards or barriers to more independent living.
Identify equipment needs.
Provide supervision to the Occupational Therapy Assistant.
Conduct an initial evaluation/assessment and report findings to the Health Care Provider and RN; develop the plan of care promptly for patients.
Document initial evaluation and treatment plan and re-evaluations of patient’s progress at least every thirty (30) days and update plan of care as necessary.
Complete comprehensive assessments and OASIS data as indicated on a timely basis.
Observe, record and report to Health Care Provider the patient’s treatment, response and progress, and promptly report significant changes.
Obtain orders to revise the plan of care.
Instruct patient/CARE GIVER in the use of alternate methods and devices for upgrading independence in activities of daily living.
Qualifications
Bachelor’s Degree and active Occupational Therapist license in the state of practice.
Work experience of at least one (1) year under a qualified Occupational Therapist; home health experience preferred.
Must have a vehicle, valid driver’s license and automobile insurance; ability to make home visits and provide OT evaluations and treatments in the home environment; complete documentation effectively and submit in a timely manner.
Home health and Home Care Home Base documentation experience.
