Overview
Maimonides is seeking an experienced
Clinical Pharmacy Specialist
in its Department of Population Health.
Responsibilities
Provide specialized pharmaceutical services to improve drug utilization and therapeutic outcomes including, but not limited to, advising prescribers on issues concerning medication therapy, drug interactions, adverse effects, and medication safety issues. Actively participate in developing and presenting educational in-service programs for pharmacists, pharmacy interns and technicians, other healthcare providers, and patients as required. Assist the Population Health department in providing Collaborative Drug Therapy Management (CDTM) which includes review, evaluation, and management of drug therapy of patients being treated by a prescriber for a specific disease or disease state, in accordance with a written agreement or protocol with a voluntarily participating physician and the policies, procedures, and protocols of the facility. (This includes all activities in accordance with legislation 6801 subsection 2.) Adhere to practice standards and guidelines as put forth by ASHP, NCQA, HEDIS, and JCAHO and comply with local, state, and federal laws and regulation applicable to services rendered. Counsel and educate patients and parents/caregivers concerning their medication during clinic hours, at the time of admission, during their hospital stay, and upon discharge to address potential drug interactions, adverse effects, and barriers to adherence. This includes demonstration of the proper administration technique of inhalers, injectables, appropriate measuring of liquid medications, and topical products. All counseling should be documented in the electronic medical record. Any concerns raised by patient may be discussed with their care team during their stay. Maintain compliance with departmental best practices regarding safe medication use and Joint Commission National Patient Safety Goals. Participate in chronic disease management with multidisciplinary teams. Foster a learning environment to train and precept future ambulatory care providers including APPE pharmacy students, PGY-1 and PGY-2 residents by teaching clinical problem solving through instruction, modeling, coaching, and facilitation.
Qualifications
Graduation from an ACPE-accredited College of Pharmacy with a BS and an advanced graduate degree (M.S., PHD, or a postgraduate Pharm.D.) or PharmD Completion of a PGY-1 pharmacy residency program Completion of a PGY-2 Ambulatory Care Pharmacy Residency Program If no residency experience, must have 5 years of clinical experience in ambulatory care and 3-5 years of experience precepting pharmacy students or residents
Board of Pharmacy Specialties certification in the area of specialty (preferred) Additional requirements: Current New York State pharmacist licensure (or eligible for licensure, but will not be able to start until licensure is obtained) BLS/PALS certification preferred At least 5 years of clinical experience in ambulatory practice (if applicant did not complete residency training) Excellent verbal, written, interpersonal, organizational, and communication skills Excellent computer skills including proficiency in preparing power point presentations and analyzing excel worksheets Knowledge of various Electronic Medical Records preferred (Allscripts, NextGen, Epic) Knowledge of a second language preferred
Pay Range
USD $104,919.00 - USD $125,000.00 /Yr.
