We're looking for a
CNA Coordinator
to join our compassionate and mission-driven team. In this critical role, you will coordinate and schedule CNA visits based on patient care plans, ensure compliance with regulatory requirements, and help create an extraordinary care experience for our patients and their families.
This position is ideal for someone who thrives in a fast-paced environment, enjoys problem-solving, and has a strong background in healthcare operations, particularly in
home health or hospice care .
Key Responsibilities:
Scheduling & Coordination
Maintain daily CNA schedules, accounting for staff availability, vacations, and absences
Schedule initial CNA visits and supervisory visits in alignment with care plans
Assign consistent CNAs to patients to ensure continuity of care
Coordinate with families, patients, and clinicians to ensure visits meet patient needs and expectations
Communicate appointment details to patients and healthcare facilities
Compliance & Workflow
Reconcile scheduled visits against visit orders to ensure regulatory compliance
Monitor and ensure timely completion of supervisory visits, providing weekly reminders to staff
Collaborate closely with CNA Supervisor and Field Clinicians to flag and resolve any issues
Assist as backup for Team Coordinators as needed
Qualifications:
2+ years of community health or healthcare operations experience
(home health/hospice preferred)
Bachelor's degree OR high-functioning Medical Assistant, CNA, Coder, or licensed healthcare personnel
Strong understanding of
medical terminology
and home health regulations
Excellent organizational and multitasking skills
Outstanding
customer service and phone communication
skills
Ability to thrive in a dynamic, patient-focused environment
Comfortable using scheduling and documentation systems (e.g., HCHB or similar EMR)
