Travel Licensed Social Worker (LSW) - $2,086 per week in Fort Pierce, FL

Number of beds in the facility is 344, excluding behavioral health and acute rehabilitation units. Patient ratios may vary: when paired with another RN, the case load may be complex with 8 to 9 patients. When focused solely on discharge planning, the social worker may handle 1:25 or up to 30 patients. Familiarity with MIDAS/Meditech, Nate, Navihealth, and Care Team is essential. All staff members are required to work every third weekend. Social workers may float to other areas, excluding trauma. Knowledge of hospital discharge planning and execution is required. Experience with multidisciplinary rounds and discharge planning is essential. Typical procedures include discharge planning, referrals, and working with families and patients. An adaptable attitude in a fast-paced environment is preferred.
