Overview
Sapphire Center for Rehabilitation and Nursing of Central Queens seeks a full-time RN Supervisor to oversee day-to-day clinical activities of health care personnel (LPNs and CNAs). The RN Supervisor interprets patient information, makes critical decisions about actions, and administers medication, wound care and other interventions as needed.
Base pay range
$47.00/hr - $49.00/hr

Sapphire Center for Rehabilitation, a skilled nursing facility located in Queens County, New York, is looking to hire clinically superb full-time RN Supervisors. The RN Supervisors will oversee the day-to-day clinical activities of health care personnel, such as LPNs and CNAs. The RN Supervisor must be able to interpret patient information and make critical decisions about necessary actions, as well as administer medication, wound care and other personalized interventions.
Sign-on bonus
We are paying a $5,000.00 signing bonus for this position (to be paid after successful completion of 90 day probationary period).
Job Duties / Responsibilities
- Completes admissions assessments and reassessment on all patients.
- Provide leadership and supervision to the nursing staff, housekeeping, and security.
- Work closely with medical support staff to ensure resident needs are met.
- Greet admitted patients and prepare them for physical examination and treatment.
- Maintain medical supplies and equipment.
- Provide education as indicated after assessing the learning needs of the patient and staff members.
- Assess the patient’s physical and psycho-social condition on an ongoing basis throughout the continuum of care.
- Utilize manual and automated information systems to improve efficiency; accurately record patient information in the electronic medical records system and patient medical record.
- Demonstrate current knowledge of equipment, instrumentation and medications.
- Act as a liaison coordinating patient care activities with nurses, physicians and allied health professionals.
- Participate in preparation and facilitation of regulatory agency site visits (DOH).
- Maintain adequate infection control standards per agency protocol and DOH standards.
- Review and report all medical, accident/incidents related to patient care and provide input into actions necessary to resolve problems.
- Participate in weekly and on-demand case conferencing with interdisciplinary team members.
- Ensure personnel adhere to established policies and procedures.
- Document changes in residents’ conditions.
- Participate in care planning meetings.
- Mentoring and supervision of LPNs and CNAs.
Requirements
- Current NYS Registered Nurse license.
- 3 years RN experience preferred.
- Current BLS Certificate.
