Summary of Position: Responsible for patient treatment and services including intake, assessment and integrative summaries, treatment planning and updates, and transition / discharge planning and referrals according to state of Colorado and federal regulations and CARF clinical standards. Responsibilities
Completes screening / intake for patient eligibility for an ASAM Level 1 outpatient treatment, and refers as appropriate Orients new clients and ensures their understanding of the program, its guidelines and opioid agonist medication Conducts addiction treatment services independently for individuals, families, and groups Conducts assessments and develops an integrative summary used in the development of the individual’s treatment plan Prepares individualized treatment plan consistent with assessment findings and refers patients to services as appropriate Re-assesses and completes treatment / concurrent reviews on patients and revises treatment plans as needed Completes transition plans and discharge summaries for discharged patients Conducts individual sessions in accordance with the treatment plan, to provide feedback, support or encouragement, address behaviors and attitudes, or family, social or personal problems Ensures presence of a release of information to protect patient privacy and confidentiality when communicating and following up with stakeholders (i.e., family, primary care physicians, other providers) Models fidelity Evidence Based Practices and counselor competencies, as required Ensures and promotes a patients' rights to privacy, confidentiality, and other legal rights Maintains timely documentation in patient records in accordance with CBHC policies, Colorado and federal (CSAT and HIPAA) regulations, and CARF standards Assists in preparation of the program for CARF accreditation and the implementation of organizational and clinical standards Maintains a working knowledge of state and federal regulations, and the CARF Opioid Treatment Program standards Qualifications
CAS Certification with bachelor’s degree or higher-level education Three years experience as a counselor for substance abuse and/or mental health adult patients in an outpatient / inpatient setting Professional Competencies
Communication: Must be able to express ideas clearly, concisely and in plain language to patients and their families Judgment: Uses clinical judgment in providing counseling and resolving treatment issues with patients and their families Opioid Treatment Knowledge: Working knowledge of ASAM Level 1 Criteria, addiction treatment and opioid agonist medication Benefits
