Participate in clinical performance improvement activities. Apply InterQual criteria to monitor admissions and stays, documenting findings per standards. Assist in compiling physician profile data related to LOS, resource utilization, and quality indicators. Report financial indicators including case mix, LOS, and readmission rates. Collaborate with multidisciplinary teams in discharge planning, assessments, and care coordination. Facilitate communication among medical staff, nursing, and ancillary services to ensure efficient care delivery. Manage patient care plans, monitor progress, and adjust as needed to ensure high-quality, cost-effective care. Coordinate with external case managers and communicate with financial counselors for reimbursement processes. Complete utilization management and document discharge planning in the system. Identify at-risk populations and initiate referrals for home health or hospice care. Manage discharge planning, including patient/family meetings and transfer arrangements. Monitor length of stay and resource use, taking actions to improve efficiency. Perform UR reviews and resolve discharge delays proactively. Use data for performance improvement and quality assurance. Qualifications

Education : Associates degree in Nursing required; Bachelor's preferred. Experience : 3 years in utilization review and discharge planning preferred. Licenses & Certifications : Valid RN license in California, BLS certification; professional case manager certification preferred. Position Summary
