Case Manager Flow Lead Responsibilities
Serve as a Lead and resource for Discharge Planners (DCP) and the multidisciplinary team by providing guidance, training, participation in Multidisciplinary Rounds (MDRs), and management of patient assignments. Assess patients' clinical and psychosocial needs through review of information, contact with patients/families, and collaboration with care teams; ensure referrals for Social Determinants of Health (SDOH) needs and identify at-risk populations using approved screening tools and reporting procedures. Initiate and facilitate referrals to specialists, clinics, home healthcare, hospice, SNF, acute rehab, LTAC, Transitional Care Management (TCM), medical equipment and supplies as indicated. Communicate with patients, families, care team members, and other parties regarding care plans, progress toward goals, concerns, and resolution of issues; document to ensure continuity of care. Seeks consultation from appropriate disciplines/departments to expedite care and assist discharge. Communicate with payers to ensure reimbursement certification for assigned patients; discuss payer criteria with the Discharge Planner and resolve payer issues with clinical staff as needed. Collaborate with Discharge Planners, care teams, patients and families in assessing and coordinating discharge planning needs; work with internal and external case managers. Ensure safe care by adhering to policies, procedures and standards within budget, including time/supply management, productivity and accuracy of practice. Assist in collecting and reporting resource and financial indicators (clinical metrics, LOS, cost per case, excess days, resource utilization, readmission rates, denials and appeals). Collect and analyze variances from plans of care and care paths with physicians and care team; collect delay data and quality metrics for performance indicators. Minimum Requirements
Education: Bachelor's Degree in Nursing or Master's Degree in Social Work. Experience: Minimum of four (4) years acute care case management experience in an acute healthcare environment; understanding of DCP for specific disease states; understanding of Social Determinants of Health (SDOH) impact on health. License: Must be licensed in the Commonwealth of Virginia to practice as a Registered Nurse (RN) or licensed as a Social Worker in Virginia or eligible to practice as a Social Worker in Virginia. Certification: Basic Life Support (BLS) for Healthcare Providers from the American Heart Association required upon start. Must have one of the following: Accredited Case Manager (ACM) or Certified Case Manager (CCM). Preferred Qualifications
Four (4) or more years of inpatient (hospital) case management experience, discharge planning, and supervisory/lead experience; experience handling medically complex cases is highly preferred. About Us and Equal Opportunity
