The Care Navigator (CN) assesses families and individuals enrolled in Medicaid for support services across social and health-related domains. The CN guides clients to enhanced services provided by Social Care Networks or existing programs and follows up to ensure services are delivered and needs are met. Assess unmet needs using the HRSN screening. Gather consent and conduct verbal screenings over the phone and in person in all 5 boroughs. Enter data into Social Care Network platforms promptly. Conduct eligibility assessments for screened individuals to determine qualification for EHRSNs. Refer qualifying members to services within their borough; refer non-qualifying members to existing services. Document all contacts and follow-ups accurately and timely across platforms. Follow up to ensure appointment compliance and monitor changes in needs due to Major Life Events within a year of initial screening. Provide technical support to staff regarding online platforms and databases. Qualifications

High School Diploma/GED and one year of relevant experience in child welfare or social services, or a related degree preferred. Experience with service coordination, linkages, and referrals to community programs. Passion for serving children and families; relevant experience preferred. Strong communication, interpersonal, problem-solving, organizational, and time management skills. Ability to work independently and as part of a team, meet deadlines, and maintain accurate records. Ability to work effectively with diverse populations and teams. Open to feedback and adaptable to program needs. Willingness to travel for in-person meetings. Commitment to safety. Education Required: High School / G.E.D. Salary Range: $24.97/hour plus education and bilingual add-ons. Employment Details
