Chatham The Reentry Program prevents individuals recently released from Illinois Department of Corrections (IDOC) facilities from experiencing homelessness when returning to their communities. Through a partnership with the Center for Housing and Health, reentry households receive subsidies for monthly rent, financial assistance for moving fees and other prohibitive costs, such as furniture and transportation, and intensive wrap-around supportive services with Trilogy. The Reentry Program will provide permanent supportive housing for individuals recently released from an Illinois prison or jail and returning to Chicago. The program prioritizes individuals with chronic illnesses, including moderate to severe mental illness, Murphy Class members (individuals with sex offense convictions), and other individuals considered “hard to house” and otherwise unable to leave an IDOC facility without a housing placement. Salary:

$27.37 - 31.47 /Hr. Position Overview:

The Senior Housing Case Manager (SHCM) will assess the client’s personal, medical, emotional, social, and environmental situation to plan for treatment course through home, community, and office visits. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance use/addiction disorders utilizing a harm reduction approach. They will manage clinical assessments and care for all clients on the caseload. The SHCM will provide at least 75% of all community support services in the community. Responsibilities Coordinate the establishment of an individual recovery plan and ongoing care with the client, client’s support system, and other care providers (e.g., parole, court mandated services, CHH, medical providers, etc.). Complete daily progress notes based on services provided to clients in a timely manner according to Trilogy’s documentation policy, project guidelines, and agency standards. Complete all necessary clinical documentation related to client care (e.g., IMCANS, LOCUS, tracking forms in Carelogic and Caseworthy). Support Re-Entry HCM with developing and honing clinical skills and collaborating to best serve clients. Provide referrals and support in linkage to additional services (food pantries, employment, therapy, etc.). Assist clients in identifying signs of de-compensation, assess for crisis situations, and coordinate stabilization as needed. Independent assessment of risk and need for higher level of care (hospital, detox, emergency services). Coordinate risk management with colleagues/supervisors and arrange two-person visits when required. Assist clients in improving independent living skills (hygiene, housekeeping, nutrition, shopping). Educate and assist clients with applying for entitlements (SSI, SSDI, Medicare, Medicaid, LINK). Support transition off master leases and access to permanent housing pathways (CHA, SRN, HUD-PSH, senior housing, etc.). Accompany clients to important appointments and provide support with housing, substance use, budgeting, social support, health, and medications. Develop partnerships with re-entry organizations in Chicago to serve client needs. Advocate for clients and empower self-advocacy when appropriate. Meet all CHH grant scopes, deliverables and timelines. Participate in monthly System Integration Team meetings with CHH, IDOC, behavioral health team members, and other partners. Communicate effectively within a multi-disciplinary team and share a caseload. Perform other related duties and/or projects as assigned.
