Remote Opportunity! Acute Care Nurse Practitioner (ACNP) in Connected Heart Failure Management Telemedicine - MedStar Health. Join MedStar Health’s growing telemedicine services as a Connected Heart Failure Monitoring and Management provider. You will provide on-demand clinical care directly to patients using our telehealth platform (MedStar video visit) in compliance with heart failure clinical guidelines under the direction of the cardiology physician team. You will effectively transition care of patients from telehealth encounters to in-person care for emergent, urgent, and scheduled follow-up needs. This program aims to improve patient access to a clinician and facilitate care within the MedStar system. The provider will join a virtual team across Maryland, DC, and northern Virginia regions. Responsibilities
Provide on-demand clinical care via telehealth to patients with heart failure as part of the connected heart failure management program. Adhere to heart failure clinical guidelines and coordinate under the direction of the cardiology physician team. Coordinate transition of care from telehealth encounters to in-person visits for emergent, urgent, and scheduled follow-up needs. Participate in a telemedicine team that serves across Maryland, DC, and northern Virginia. Qualifications
ACNP Certification At least 3 years of Nurse Practitioner experience in cardiology, with heart failure as a specialty (preferred) Reside in or be able to obtain licensure in MD, DC, or VA; at minimum one of MD, DC, or VA required at time of hire Employment Details
Full-time remote position, Monday through Friday, 8-hour flexible days 8-12 week orientation, including 1-2 days per week on-site in the facility Compensation & Benefits
