What You’ll Do
• Complete and coordinate MDS assessments • Develop and update individualized care plans • Monitor resident progress and support QAPI efforts • Communicate with residents, families, and providers
What You’ll Bring
• Current RN license • Knowledge of MDS 3.0, RAI, and care planning • Strong communication and organizational skills • RAC-CT certification preferred

Pay up to $84,000 annually + merit increases
