Posted Tuesday, March 19, 2024 at 4:00 AM Job Responsibilities: Interviews patient and family members to obtain information about home environment, family relationships, and health history. Performs accurate social work assessment. Documents findings in the patient's electronic health record. Provides support services and counseling within the social worker's scope of practice to patients with psychosocial issues related to their illness, family illness, and grief reactions. Provides for multi-disciplinary approach to treatment plan, coordinating with and providing timely feedback to other medical staff. Reviews socioeconomic issues in individual patients' cases and assists with explanation of how they impact the patients' care. Educates and supports patients and families with exploration of care options, including availability and eligibility for home health services, hospice, assisted living, and skilled nursing facilities. Assists with preparation of admission paperwork. Educates and supports patients and families in options and services available related to the healthcare system, e.g. Medicare, Medicaid, Medicare Part D, vocational rehabilitation, etc. Refers patients to such services as appropriate. Maintains current information about locally available healthcare and community services. Serves as a liaison between the department and various community service organizations. Performs other duties as assigned or requested. Education:
Bachelors degree in Social Work required. Masters degree preferred. Experience:
Two years experience preferred.
