Position overview: Quality Practice Advisor responsible for coordinating performance improvement activities to engage, educate, and manage activities to improve health outcomes for Healthmap Solutions members. Partners with Care Navigation team to improve the Kidney Health Management clinical program. Company background

Develop a trusting partnership with clinical providers and practice locations Identify opportunities to improve health outcomes for Healthmap Solutions members based on provider-specific data Educate and communicate best practices to address identified areas of provider low performance Assist in identifying opportunities for process improvement in provider office workflows Partner with physicians and staff to identify members who would benefit from Care Navigation support Support operational and clinical stakeholders in identifying, developing, and executing process improvement initiatives Improve provider performance in areas such as Healthmap Solutions Interventions, Care Gaps, and Chronic Conditions Educate providers on HEDIS measures and open Care Gaps Support the development and implementation of quality improvement interventions Educate provider offices on Chronic Kidney Disease, End Stage Renal Disease, Renal Replacement Therapies, etc. Build cross-functional relationships with internal departments and provider partners Collaborate with Care Navigation to enhance care coordination for Healthmap Solutions members Maintain thorough documentation of all provider meetings for consistency and coordination Maintain documentation in compliance with NCQA standards Deliver timely reports to internal and external stakeholders Perform other related duties as assigned Qualifications
Bachelor’s degree required Active, unrestricted RN license required Basic Life Support (BLS) certification required Advanced Cardiovascular Life Support (ACLS) certification (based on role) 3 years of experience in a health care or managed care setting 3 years of experience in claims or gap closure campaigns (preferred) 3+ years of progressive experience in healthcare services, clinical operations, quality, or care management Prior experience building and managing relationships with health care providers (preferred) Valid, unrestricted driver’s license and automobile insurance as required by law; regular travel within assigned region Same state residency required Compliance and travel: Must comply with organization health screening and immunizations requirements; travel is heavy (over 50%) to support practices in an assigned geographic market. Travel
Heavy travel, 50% or more, to support practices in the assigned region. Compensation and benefits
Base pay range:
$105,000.00/yr - $138,000.00/yr . Compensation may vary by market/region and experience. Eligible for bonus incentives. Benefits include paid time off, medical, dental, vision, disability insurance, 401(k) with match, and other voluntary benefits. Legal and equal opportunity
