Ascension St. Vincent Employer Solutions - Wellness Clinic is seeking a Primary Care Advanced Practice Practitioner (NP or PA) to join our team. Our employer-based clinics focus on improving the health and wellness of clients’ employees and dependents by providing accessible services where they work and live. Position Highlights
Full Time position Schedule: 4 1/2 days per week, Monday - Friday (no weekends/holidays) Clinic locations: Evansville and Princeton Responsibilities
In collaboration with a physician, expand individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions. Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care. Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources. Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources. Document delivery of health care and nursing processes in accordance with specific standards and network nursing policy. Requirements
Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. One or more of the following required: Nurse Practitioner credentialed from the American Nurses Credentialing Center (ANCC) obtained within 6 Months (180 days) of hire date or job transfer date. Nurse Practitioner credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date. Nurse Practitioner credentialed from the National Certification Corporation (NCC) obtained within 6 Months (180 days) of hire date or job transfer date. Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date. Education
Master's degree of Nursing required. Benefits
