DaVita Integrated Kidney Care (DaVita IKC) is one of the largest end-to-end kidney care providers; we manage $5.5B+ in annualized medical spend through value-based partnerships. We provide comprehensive care and complex chronic condition management to 80k+ patients nationwide, whether they are in the early stages of kidney disease, transitioning to dialysis, seeking a kidney transplant or receiving life-sustaining dialysis. This Director-level role is tasked with advancing IKC’s value-based care programs, focusing on developing nurse practitioner–led interventions that improve patient care, clinical collaboration, operational efficiency, and cost-effectiveness. The ideal candidate will be highly organized, self-driven, and comfortable operating independently in ambiguous environments. This position will lead process improvements, drive strategic initiatives, and work closely with product and clinical teams to boost NP efficacy, advance care team integration, and deliver measurable outcomes.

Pay and Location

Responsibilities Drive IKC’s value-based care priorities by creating and scaling NP-specific interventions to reduce total cost of care, improve clinical outcomes, and enhance patient experience.

Actively support advancement of IKC programs by driving strategic projects focused on care delivery transformation: improving patient care, enhancing care team collaboration, and optimizing NP efficiency.

Collaborate across clinical operations, analytics, product, and innovation teams to co-design workflows, pilot NP-led initiatives, and implement scalable models.

Align with product development teams to ensure solutions meet clinical needs and drive adoption at scale.

Employ process improvement methodologies to re-engineer clinical and operational workflows, addressing gaps and improving efficiency.

Lead change management efforts to ensure NP interventions and operational solutions are adopted smoothly and sustainably.

Establish, test, measure, and refine pilot programs utilizing NP-led strategies across IKC workflows, including KPIs, charters, and ROI evaluations.

Leverage analytics to monitor performance, extract insights, and iterate program design.

Produce compelling executive-level communications: presentations, dashboards, and briefings that drive alignment and accountability.

Serve as a persuasive thought partner and advisor to senior leadership and external stakeholders.

Provide leadership to cross-functional project teams—fostering accountability, inclusion, continuous improvement, and results-focus.

Minimum Qualifications Healthcare Experience: Minimum of 5 years in the healthcare sector, with experience in clinical operations, healthcare strategy, or value-based care environments.

Education: Bachelor’s degree required; MBA preferred.

Operational Skills: Strong background in process improvement, strategic frameworks, and planning within healthcare.

Stakeholder Management: Proven ability to work across diverse teams and navigate competing priorities.

Personal Attributes: Highly organized, self-motivated, disciplined, and capable of working autonomously.

Ambiguity & Change Management: Comfortable leading in rapidly evolving, ambiguous environments.
