Summary: The Clinical Pharmacist Specialist provides direct and indirect patient care, education, medication safety, performance improvement, and consultation. The role includes operational and clinical pharmacy services to a general patient population and demonstrates Rush mission, vision and values and adherence to Rush policies and procedures. Required Job Qualifications Bachelor of Science in Pharmacy. Illinois Pharmacist License. Minimum of four years of Clinical Pharmacy experience OR PGY1 residency completed within the last seven years, including Clinical/Specialty Pharmacy experience. Knowledge in intravenous admixture services, intravenous drug therapy and chemotherapy. Ability to work effectively with others to provide safe and effective drug therapy. Excellent problem solving skills and ability to work independently and respond quickly to problems. Preferred Job Qualifications BLS Certification. ACLS Certification (or to be obtained upon hire). Board of Pharmaceutical Specialties Board Certification. Doctor of Pharmacy degree and residency certificate. Experience and knowledge in specialty drug therapies. Responsibilities Order verification, discharge counseling, projects, and education of other healthcare professionals. Evaluate and verify Epic physician orders for therapeutic appropriateness, dosing, drug interactions, allergies, route and rate of administration, and therapeutic redundancy. Clarify physicians\' orders when necessary through direct and timely contact with prescribers. Provide patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff. Document clinical interventions per the department\'s current method. Daily review of pharmacy reports including pharmacokinetic monitoring and patient-specific reports as required. Conduct monthly nursing unit inspections of medication storage areas as assigned. Supervise pharmacy technicians involved in preparation and delivery of medications, coordinating workflows with delivery schedules. Coordinate medication ordering, preparation, dispensing, and administration for the patient populations outlined above daily. Implement all drug use policies and formulary decisions of the Pharmacy and Therapeutics committee. Participate in SE (ADR and medication error) reporting programs. Complete clinical and operational activities as designated by management (e.g., Pyxis inventory optimization, process improvement projects). Precept pharmacy students and residents (PGY1 and PGY2) in clinical and operational areas. Assess and evaluate medical and drug literature to compare regimens and uses for new or existing treatments. Implement medication safety initiatives and develop operational and clinical projects as needed. Act as a liaison with other departments for collaborative research activities. Evaluate and monitor drug therapy in ambulatory care in collaboration with physicians, nurses and other professionals. Participate in professional organizations.
