Master's degree in counseling or social work. Certifications, Licenses or Registration Required
Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) in the State of Mississippi Knowledge, Skills & Abilities
Knowledge of human behavior and performance; psychosocial methods; assessment and treatment of behavioral and affective disorders; communication and interpersonal skills; computer literacy; data gathering and analysis; problem solving. Responsibilities
Conducts and documents detailed psychosocial assessments/evaluations to interpret social, environmental, and medical needs of the patient/family; meets the needs of transplant applicants and donors and/or level II evaluations to determine mental illness/mental retardation discharge needs; documents per departmental guidelines and standards, prepares reports and maintains records. Reassesses, monitors, and documents needs and interventions. Ensures medical coverage by securing appropriate documentation and materials. Evaluates the effectiveness of the plan of care and recommends alternatives as needed. Provides or supervises plan of care interventions including resource/referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling; collaborates with the healthcare team and involves the patient/family in plan development and execution. Collaborates with other healthcare team members to facilitate interdisciplinary care and positive outcomes. Recommends referrals and allocates community resources as appropriate. Participates in performance improvement initiatives including data collection/analysis, continuing education, development/editing of policies and procedures, and committee involvement; maintains reports and attends relevant meetings. Maintains knowledge of community agencies and relevant medical/legal issues impacting patient care; stays current with social work and health care developments. Acts as a liaison to financial counseling to assist patients with state assistance, charity care, or other programs. Occasional exposure to high noise levels, heat/cold, potential biohazards, and other occupational hazards. May involve extended hours, travel to offsite locations, and physical activities such as lifting up to 75 pounds, bending, climbing, standing, and driving as required by the job. Time Type
