Job Title: Nurse Practitioner - Sign On Bonus Offered at

Monogram Health . Monogram Health is seeking skilled Nurse Practitioners and Physician Assistants to join an integrated Care Team. The Advanced Practitioner works with a Registered Nurse and a Social Worker to support patients with multiple serious diseases, aiming to improve quality of life in the home and slow the progression of kidney disease. Responsibilities

Conduct assessments, including comprehensive annual wellness exams, in patients' homes or via virtual visits. Counsel and educate patients and families about benefits and programs to support healthier lives. Document clinical information: chief complaints, diagnoses, medical history, review of systems, examinations, medications, allergies, assessment, and plan. Coordinate care with primary care providers, specialists, and ancillary services. Complete all documentation and paperwork in a timely manner; maintain quality of care standards. Work remotely in-state (not office-based) with periodic training/meetings outside of that state as required. Deliver evidence-based, timely care that reduces avoidable hospitalizations, maximizes quality of life, and centers on patient health and satisfaction. Prescribe medications, order tests, and collaborate with Monogram physicians. Contribute to closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing. Participate in integrated care team meetings and support enrollment in educational resources as needed. Develop treatment plans and implement care plans aligned with patient needs and appropriate plans of care. Follow up and evaluate patient status; perform duties as assigned. Position Requirements

Bilingual (English/Spanish) highly preferred Basic Life Support (BLS) certification required; onboarding supports certification Active and unrestricted RN and NP or PA licenses Board certification (NP: ANCC/AANP; PA: NCCPA) Current and unrestricted DEA certificate Ability to work autonomously without direct supervision Access to transportation, valid driver’s license, and car insurance Proficiency with medical instruments and equipment required for the role Experience with computer-based data management and medical records systems Strong verbal and written communication with patients, families, physicians, and community partners Understanding of federal and state regulations including HIPAA and OSHA 2+ years of direct patient care; managed care/IPA/health plan experience Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home About Monogram Health
