Primary Care Physician

Conviva Senior Primary Care . This position is located at our South Zarzamora clinic in San Antonio. The total compensation package (base pay + bonus) could exceed $300K depending on experience and location. Base pay range: $203,400.00/yr - $299,500.00/yr. Responsibilities
Evaluates and treats center patients in accordance with standards of care. Follows level of medical care and quality for patients and monitors care using available data and chart reviews. Coordinates patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment and home health care. Actively participates as a medical expert with the care team through daily huddles. Supports a cooperative practice environment and professional demeanor with the Regional Medical Director and Center Administrator. Completes medical record documentation in a timely manner with a quality-based coder to optimize coding specificity. Follows policy and protocol defined by Clinical Leadership. Meets with the Regional Medical Director about quality of care, outcome data, policy, procedures and records issues. Participates in growth opportunities for new or existing services within the Center. Participates in the local primary care on-call program as needed. Ensures compliance with licensing, certification and accrediting bodies. Spends 100% of time clinically focused on direct patient care, including patient-facing time and related administrative tasks. Qualifications
Current and unrestricted medical license or willingness to obtain in the state of TX Graduate of an accredited MD or DO program Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine (preferred) Active and unrestricted DEA license Excellent verbal and written communication skills Strong interpersonal skills and ability to work within an integrated team-based care model Willingness to learn/adapt to a value-based care setting Superior patient/customer service Basic computer skills, including email and EMR TB screening may be required if selected for this role Preferred Qualifications
Medicare Provider Number Medicaid Provider Number 2-5 years of directly applicable experience (preferred) Experience managing Medicare Advantage panel and knowledge of Medicare guidelines, HEDIS indicators Bilingual in English and Spanish Additional Information
