Harbor Health is seeking a mission-driven Family Nurse Practitioner to join the Primary Care Department at the Daniel Driscoll Neponset Health Center in Dorchester, MA. Role: Full-time Nurse Practitioner provides care appropriate to the patient population served, including routine health maintenance, management of chronic conditions, and management of acute illness. Hours may include evening shifts and Saturday rotation based on clinic needs. Responsibilities

Serves as a primary care provider for a panel of patients. Organizes and manages the care of primary care patients, including outpatient visits, communications with patients by mail, email or phone, inpatient care, referral for specialty care, patient education for self-management, assessing health risks, promoting disease prevention, and practicing evidence‑based medicine. Provides routine health maintenance with interval well visits, including history and physicals, risk assessment, cancer screening, and health promotion education. Follows up on problems and makes referrals for additional services as needed (nutrition, social services, behavioral health, specialty care). Provides timely and expert care for acute illnesses. Manages common chronic illnesses with appropriate patient education and follow-up. Participates in an on-call system with Center and affiliated health centers to provide 24-hour provider access for patients. Participates in evening/saturday/holiday clinic schedules for face-to-face care, follow-up visits, acute care and other clinical services. Participates in provider staff meetings, department meetings, trainings, care team meetings, and quality improvement sessions. Participates in managed care plans and reinforces patient understanding of plans; makes appropriate, cost-efficient referrals and referrals to participating specialists; supports systems for managed care programs. Updates Electronic Medical Record (EMR) with accurate coding and timely documentation of care; maintains current problem list, medication flow sheet, immunization record and other pertinent information. Requirements

Completed accredited Nurse Practitioner program in Family Practice (highly preferred) or Adult Medicine. Current MA NP License, Family Practitioner, Board Certified, Current DEA, MCSR, and Basic Life Support for Healthcare Providers (BLS) required. Computer literate (MS Office, Windows); experience with EMR (EPIC/OCHIN highly desired). 2 or more years of NP experience; bilingual preferred. Experience with a community health center population and practice setting preferred.
